Social Prescribing Link Worker – Mid Chiltern PCN

Hours: Full-time

Salary: from £24,907 Dependant on experience

Contract Type: Permanent

An exciting opportunity has arisen to join the Mid Chiltern Primary Care Network as a Social Prescribing Link Worker.  Mid Chiltern PCN is one of 13 PCNs in Buckinghamshire. It covers the population of its 5-member practices; Rectory Meadow Surgery, Amersham Health Centre, John Hampden Surgery, Prospect House Surgery, Hughenden Valley & Chequers Surgeries with a combined registered population of just over 43,500

We are looking for someone that perhaps has worked within the NHS previously, maybe as a receptionist or an administrator and is experiences in use of clinical systems (we use EMIS). You might be looking to change your role and make a real difference in patients’ lives by supporting and signposting patients through referrals of non-medical pathways to support and link patients to resources within the local community. You will become our expert of local resources that patients may need support to access.

Clinical Pharmacist – Mid Chiltern PCN

Mid Chiltern Primary Care Network have an exciting opportunity for a Clinical Pharmacist to join their dynamic team. We are looking for someone enthusiastic about providing high quality care to our patients and becoming an integral part of our growing clinical pharmacy team.

The Clinical Pharmacist will be a patient facing role with plenty of opportunity to utilise your clinical skills to improve the health of the local community; this will include managing long-term conditions, supporting hospital discharge prescribing arrangements, providing specific advice for those on multiple medications, clinical audits to improve patient care, and supporting the GP practices involved to develop the clinical pharmacist role.
